As a result of the COVID-19 pandemic and the need to strengthen and promote health protection to patients, staff and members of the public attending in the hospital, prior to the first day of employment with the Hospital, all successful candidates are required to provide proof of COVID-19 vaccination (full vaccination is required) from the Ontario Ministry of Health website or other authorized source.
An individual is fully vaccinated if they have provided proof of vaccination in the form of a receipt from COVaxON (the Ministry’s provincial system for COVID-19 vaccination information) and 14 days have elapsed from the final dose.
In the event that you are unable to be vaccinated as a result of a ground protected under the Human Rights Code, you may submit a written explanation of the ground and any supporting documentation to determine if you are exempt from this requirement.
Position Summary Dietitian,ReactivationCareCentre Your goal in this position is to provide excellent patient and family driven care, in a supportive and culturally sensitive manner.
You will screen and assess patients requiring nutritional care and developing appropriate nutritional plans of care.
You will recommend/interpret therapeutic diet prescriptions and nutrition support plans by utilizing best practice guidelines.
You will chart assessments, plans and progress in the patient’s medical record.
You will participate at medical team meetings and act as liaison and resource person for nutrition for nursing and medical staff.
You will counsel individuals and groups in therapeutic nutrition and nutrition requirements.
You’re a great relationship builder, and foster close contacts with peers across the entire organization.
You have great interpersonal skills, and effortlessly connect with physicians, staff, patients, and families.
This position supports the hospital’s academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation.
This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for staff, physicians, volunteers, learners, patients and families.Dietitians are:

On a practical level, you will Provide client-centered assessments of emotional, physical, cognitive and social needs and capabilities of patients as well as their familiesPrepare nutrition care plans that meet patient needs in collaboration with the inter-professional teamsDocument written assessments and progress notes on patient health records; monitor, reassess and evaluate for optimal outcomes, which are communicated to other health care team members, as requiredRecommend and implement therapeutic diets and nutrition support orders by utilizing best practice guidelinesAdvocate for the patient/family and community to ensure the patients’ rights are recognized and appropriate services are created/provided in collaboration with the inter professional teamFormulate and facilitate the discharge plan with patient, family, and health care team utilizing appropriate community resources; recognizing current needs, strengths, and limitations of patient and family resourcesParticipate in medical team meetings and inter-professional rounds, and acting as a liaison and resource person for all staff.Counsel individuals and groups in therapeutic diet educationSupport the education of nutrition students by precepting clinical rotations and evaluating them according to the ICDEP competenciesMaintain excellent knowledge of hospital & community resources, i.e.
Home and Community Care Support Services, Social Assistance, Ontario Disability program, Drug Programs, etc.Promote best practice and evidenced-based care through the implementation of initiatives aligned with the organization’s strategic direction.
Support in the development of program/unit goals, objectives, policies, procedures and quality assurance programsParticipate in continuing education activities for self, peers and members of the inter-professional health care teamAttend and actively participate in Clinical Nutrition Professional Advisory Committee meetings and provide Clinical Nutrition representation on corporate/unit committees, as requiredSupport the hospital’s mandate by advancing care through teaching and learning and/or transforming care through research and innovation.
Qualifications Successful completion of a 4-year degree in human nutrition and dietetics from a university program accredited by the Partnership for Dietetic Education and Practice (PDEP) or recognized equivalentSuccessful completion of an accredited dietetic internship or equivalent supervised practical trainingMember in good standing with the College of Dietitians of Ontario (CDO)1 year related clinical experienceProficient with computers, MS Word, internet searches, data entry and patient information systems.Bilingualism in French/English an asset What We Offer Working at NYGH means working with a dynamic team of fellow healthcare providers, staff, and volunteers in one of Canada’s leading hospitals.This is a Part-Time Regular position with 8 hour shifts, day shifts, including weekends, in the ReactivationCareCentre at NYGH Finch Site, 555 Finch Ave W We offer a highly competitive total compensation package which includes pay in lieu of benefits, vacation pay and the opportunity to join the Hospital Pension Plan.FOR EXTERNAL APPLICANTS ONLY:
